Convenience Store, Grocery & Gift Shop, Medical Gas Outlet & Vending Machines expire 1/31 Annually
 
Renewal forms for the 2010-2011 registration period were mailed the week of November 16, 2009.  Your location may not sell any nonprescription medications after January 31 until your registration has been renewed.
 
If your renewal is postmarked after December 31, 2009 you will be subject to the delinquent fee as noted on your renewal form.  
 
If you have not received your renewal notice by December 1st,  contact our office at 971-673-0001 or email us at pharmacy.board@state.or.us.  We will not be mailing duplicate forms unless requested.
 
PLEASE NOTE:   If you have submitted your payment and renewal form, your cancelled check is your receipt.  You, or your distributer, can check the status of your license on our new, real-time, online verification system.

If you currently hold a Certified Oregon Pharmacy Technician License please do not complete a new application.  

To avoid the $20.00 delinquent fee, you must renew your license prior to the August 31st postmark deadline.   The delinquent fee applies to all applications postmarked on September 1, 2009 or later.   If you do not receive your renewal form by August 1, please contact our office for a duplicate renewal form.

Once you submit your payment and renewal form, your cancelled check is your receipt.  You, or your employer, can check the status of your license on our real-time, online verification system.
 
If you have had an address or employment change since the last renewal cycle, and you haven´t updated your information with the Board, please submit an address change form, or submit your information online. Be advised, any change of address, or change of employement must be submitted to the Board within 15 days of the change.
 
CE INFORMATION FOR CERTIFIED PHARMACY TECHNICIANS: 
The Board will accept one hour of CE law from either the Accreditation Council for Pharmacy Education (ACPE), Continuing Medical Education (CME)  or Board approved programs.  The law CE programs may contain information on State or Federal regulations such as DEA, FDA, HIPPA or Medicare part D.  Some other suggested websites to visit are www.powerpak.com, www.cecity.com and www.rxschool.com.  The Board of Pharmacy also has the one hour CE Law exam available on our website annually.

PLEASE NOTE:   If you are currently licensed as an Oregon preceptor, please mark “yes” or “no” if you wish to renew.  If you are not licensed as an Oregon preceptor and are employed outside the State of Oregon, leave this section blank.  It is the Oregon Board of Pharmacy’s policy not to issue preceptor licenses to pharmacists who are not currently working in the State of Oregon as this license is only valid within the State.
 
As a reminder, preceptor licenses converted to a 1 year license with an expiration date of June 30th.  If you mark “yes” to have your Oregon preceptor license renewed, you will be issued a new license showing the new expiration date of June 30, 2010 even if you have previously been granted a license with an expiration date of  2010 or 2011.

To expedite processing of your application, be sure to include the following:
 
  • A copy of your driver’s license or state ID,
  • A copy of your national certification or the letter stating you have passed the exam,
  • A new, original passport regulation photograph taken within the last 6 months,
  • A check or money order for $35.00.
 
 

Read the instructions on the form carefully.
 
Truthfully answer the moral turpitude questions on pages 2 and 3. If you have already provided documentation for any “yes” answers to the Board, you may write in that we have the records on file, and you do not need to provide it again. You do have to disclose (answer “yes” for) any and all old incidents again.
 
Please allow 6 to 8 weeks’ processing time for your license. We must be able to verify your national certification on PTCB’s or ICPT’s websites. Be aware that it can take 8 weeks or more before your certification may be posted on PTCB’s or ICPT’s websites. Plan ahead, and test early! Your application will be held until we are able to verify your certification.
