Many forms are designed to be completed online. These "fill-in-able" forms may look slightly different when printed.
PERS provides all online forms and publications in PDF format. You can open them using Adobe Reader© software. Please ensure you have the most recent version of Adobe Reader©.

Most forms can be filled out on your computer. Simply use the "tab" key to move from one field to the next. You can also use your mouse to move from field to field. To do this, position the cursor in the field and click with your mouse.
Print out the form before you fill it in online to make sure your printer has the capability of printing the form correctly.
Each form has its own instructions. Make sure you read these completely, follow all instructions, and sign and date where indicated. Incorrectly completed or unsigned forms will cause a delay in processing your information.
