Oregon State Athletic Commission - Federal ID
Dear Federal ID Applicant,
 
To become a professional boxer you must first be registered with the national database approved by the Association of Boxing Commissions (Fight Fax, Inc.).  You must register through the Commission from the state in which you reside.  If you reside in Oregon please submit to the Oregon State Athletic Commission the following completed documents: 

Federal ID Application
Association of Boxing Commissions Health Disclosure

Please also submit a copy of two forms of identification. 
 
You can also find here for your review the Association of Boxing Commissions Boxers Bill of Rights.  There is important information in this document related to your rights as a professional boxer.
