The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 62 nurses or nursing assistants during its September 16, 2009 Board Meeting.
Baker County
Benton County
- The Board reprimanded the Registered Nurse license of Laurel L. James (095003052RN), of Philomath, Ore., for failing to answer questions truthfully and misrepresentation during the licensure process.
Clackamas County
- The Board suspended the Registered Nurse license and Family Nurse Practitioner certificate of Carla R. Anderson (200242827RN/200250170NP), of Wilsonville, Ore., for two-years with conditions, for practicing nursing when unable due to a physical impairment, and practicing while impaired.
- The Board suspended the Practical Nurse license of Linda K. Givens (098003027LPN), of Molalla, Ore., for 30 days with conditions, to be followed by probation for two-years, for failing to take action to preserve client safety, assigning persons to perform functions for which they are not prepared, failing to communicate information regarding client status, and failing to conform to the essential standards of acceptable nursing care.
- The Board reprimanded the Registered Nurse license of Jacqueline H. Glenn Schoenheit (200740091RN), of Lake Oswego, Ore., with conditions, for failing to take action to promote client safety and failing to conform to the essential standards of acceptable nursing practice.
- The Board accepted the voluntary withdrawal of Charles N. Miller’s Registered Nurse application for willful fraud during the licensure process. He resides in Gladstone, Ore.
- The Board accepted the voluntary withdrawal of Bonnie M. Plumlee’s CNA application for violation of a Board Order. She resides in Oregon City, Ore.
Clatsop County
- The Board reprimanded the Practical Nurse license of Annie M. Smith (200630258LPN), of Seaside, Ore., with conditions for failing to implement the plan of care, and failing to administer medication.
Columbia County
- The Board reprimanded the nursing assistant certificate of Tammy J. Roberts (200510962CNA), of Vernonia, Ore., with conditions, due to incomplete recordkeeping.
Curry County
- The Board suspended the nursing assistant certificate of Shanna M. Larios (200811765CNA), of Brookings, Ore., indefinitely for failing to cooperate with the Board during an investigation.
Deschutes County
- The Board reprimanded the Registered Nurse license and Psychiatric/Mental Health Nurse Practitioner certificate of Donald S. McFerran (081046617RN/081046617N6), of Bend, Ore., with conditions, for failure to conform to the essential standards of acceptable nursing practice.
- The Board suspended the nursing assistant certificate of Kelli M. Hutchinson (200711429CNA), of Redmond, Ore., for a minimum 14-days for failing to cooperate with the Board during an investigation, to be followed by two-years probation for using intoxicants to the extent she was unable to perform CNA duties safely.
- The Board reprimanded the Practical Nurse license of Jammie L. Treon (200930417LPN), of Redmond, Ore., failing to answer questions truthfully and misrepresentation during the licensure process.
Douglas County
- The Board reprimanded the Registered Nurse license of Teresa J. Hartman (094006397RN), of Roseburg, Ore., for incomplete recordkeeping and possessing unauthorized prescription drugs.
- The Board denied the CNA application of Gabriell Rowley of Roseburg, Ore., for failure to provide documents requested by the Board and failure to cooperate with the Board during the course of an investigation.
Hood River County
- The Board denied the Practical Nurse application of Cindy L. Knight, of Hood River, Ore., for not meeting the Oregon PN examination requirements for licensure.
Jackson County
- The Board suspended the Nurse Midwife Nurse Practitioner certificate of Bunny Day Lewis (091006559N5), of Ashland, Ore., for 180-days for prescribing medications to herself, obtaining prescription drugs without authorization, and failing to communicate information regarding client status to members of the health care team.
- The Board accepted the voluntary surrender of Paula L. Schranz’s Registered Nurse license (078041349RN) for inaccurate recordkeeping, unauthorized removal of money from the workplace, and failing to conform to the essential standards of acceptable nursing practice. She resides in Medford, Ore.
- The Board reprimanded the nursing assistant certificate of Amber D. Russell (200811254CNA), of Medford, Ore., with conditions, for failing to respect client rights, client abuse and client neglect.
- The Board reprimanded the Nurse Practitioner certificate of Justin E. Henderson (200950142NP), of Medford, Ore., for failing to answer questions truthfully and misrepresentation during the licensure process.
Klamath County
- The Board suspended the Registered Nurse license of Beverly A. Pate (094000412RN), of Keno, Ore., for 30 days, with conditions, for inaccurate and incomplete recordkeeping, altering a client record, and failing to conform to the essential standards of acceptable nursing practice.
- The Board reprimanded the Registered Nurse license of Betty J. King (086000130RN), of Merrill, Ore., for failing to answer questions truthfully and misrepresentation during the licensure process.
Lane County
- The Board suspended the Registered Nurse license of Toby S. Knight-Meigs (096006766RN), of Eugene, Ore., for 60 days with conditions, due to incomplete recordkeeping, failing to communicate information regarding client status, and failing to conform to the essential standards of acceptable nursing care.
- The Board reprimanded the nursing assistant certificate of Mary A. Lee (000032117CNA), of Eugene, Ore., with conditions, for client neglect, and failing to competently perform CNA duties.
- The Board placed the Registered Nurse license of Patricia M. Morrison (089003022RN) of Springfield, Ore., on probation for two years for previous discipline in Alaska and using intoxicants to the extent she was unable to practice nursing safely.
Lincoln County
- The Board reprimanded the Registered Nurse license of Angelika M. Schmoll (085074549RN), of Depoe Bay, Ore., with conditions, for failing to maintain professional boundaries with a client.
Linn County
- The Board accepted the voluntary surrender of Helen M. Culp’s Registered Nurse license (085074456RN) for failing to comply with the terms and conditions of a Board Order. She resides in Shedd, Ore.
- The Board placed the nursing assistant certificate of Jolynn N. Soderholm (200312728CNA) of Albany, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.

- The Board accepted the voluntary surrender of Janet R. Bonogofski’s Registered Nurse license (088003226RN) due to a physical condition that prevents her from practicing safely. She resides in Keizer, Ore.

- The Board reprimanded the Registered Nurse license of Mark M. Yount (200641325RN), of Portland, Ore., with conditions, for failing to implement the plan of care, and failing to conform to the essential standards of acceptable nursing practice.
- The Board reprimanded the nursing assistant certificate of applicant Tifani Maddox, of Portland, Ore., for failing to answer questions truthfully and misrepresentation during the licensure process.
- The Board reprimanded the nursing assistant certificate of Vera Y. Lashier (200710867CNA), of Portland, Ore., for failing to answer questions truthfully and misrepresentation during the licensure process.
- The Board revoked the Registered Nurse license of Wendy A. Cooper (094006087RN), of Portland, Ore., for violating the terms and conditions of a Board Order and failing to cooperate with an investigation.
- The Board accepted the voluntary withdrawal of Michael J. Gerton’s CNA application for using intoxicants to an extent that he is unable to perform CNA duties safely. He resides in Portland, Ore.
Polk County
- The Board denied the Practical Nurse application of Shannon M. Meeker, of Independence, Ore., due to convictions for Criminal Mistreatment.
Union County
- The Board suspended the nursing assistant certificate of Becky L. Ronnebaum (200911194CNA), of La Grande, Ore., indefinitely for failing to cooperate with the Board during an investigation. The Board also reprimanded her certificate, with conditions, for client neglect and failing to competently perform CNA duties.
Washington County
- The Board reprimanded the Registered Nurse license of Aaron b. Jolley (200942652RN), of Beaverton, Ore., for failing to answer questions truthfully and misrepresentation during the licensure process.
- The Board reprimanded the nursing assistant and medication aide certificates of Heang H. Chan (000014861CNA/000014861CMA), of Beaverton, Ore., for client abuse, failing to administer medications as ordered, and failing to document medications.
- The Board suspended the Practical Nurse license of Michelle Evilsizer (200230297LPN), of Beaverton, Ore., indefinitely for failing to cooperate with the Board during an investigation.
Yamhill County
- The Board emergency suspended the nursing assistant certificate of Sean N. Cluver (200911363CNA), of McMinnville, Ore., indefinitely for engaging in sexual misconduct in the workplace and client abuse.
- The Board suspended the nursing assistant certificate of Joeen M. Blakley (200310495CNA), of McMinnville, Ore., for 15 days, with conditions, for client abuse.
- The Board reprimanded the nursing assistant and medication aide certificates of Jennifer A. Hays (200011166CNA/200220054CMA), of Newberg, Ore., for failing to answer questions truthfully and misrepresentation during the licensure process.
Out of State
- Florida—The Board reprimanded the Registered Nurse license of Kathryn A. Powell (200440528RN), of Roseburg, Ore., for altering a client record.
- Nevada—The Board reprimanded the Registered Nurse license of Judette A. Lawrence (200542343RN), of Henderson, Nv., with conditions, for failing to take action to promote client safety and failing to implement the plan of care.
- Washington—The Board suspended the nursing assistant certificate of Rachel L. Gray (200610410CNA), of Longview, Wash., indefinitely for failing to cooperate with the Board during an investigation.
