The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 33 nurses or nursing assistants during its November 14, 2007, disciplinary Board Meeting.
Benton County
- The Board suspended Helen K. China’s Registered Nurse license (200140640RN) for 30-days, to be followed by probation for no less than one-year, for modifying the standards of care that jeopardized patient safety. Ms. China resides in Corvallis, Ore.
Clackamas County
- The Board placed the nursing assistant certificate of Joshua R. Kell (000034642CNA) on probation for two years, for using intoxicants to the extent that he is unable to perform CNA duties safely. Mr. Kell resides in Clackamas, Ore.
- The Board imposed a $3,900 civil penalty on the Registered Nurse license of Marie A. Zeller (000031522RN) of Wilsonville, Ore., for fraud and practicing without a current nursing license.
Clatsop County
- The Board placed the nursing assistant certificate of Lori Ann Paich (200712477CNA) on probation for two years for using intoxicants to the extent that she is unable to perform CNA duties safely. Ms. Paich resides in Astoria, Ore.
- The Board accepted the voluntary surrender of Kirsten J. Massebeau’s Registered Nurse license (200440393RN) for using a controlled substance to the extent that she is unable to practice nursing safely. She resides in Cannon Beach, Ore.
Coos County
- The Board suspended the Registered Nurse license of Dianne L. Elton (090006851RN) for 30 days, to be followed by one-year of probation, for abusing a client and using profane language in the presence of a client. She resides in North Bend, Ore.
Jackson County
- The Board placed the Registered Nurse license of Susan J. Musser (098006187RN) on probation for two years, effective following her successful completion of a re-entry program, because of chemical dependency issues. Ms. Musser resides in Ashland, Ore.
- The Board reprimanded the Registered Nurse license of Denise M. Strusz-Rice (086007019RN) of Medford., Ore., for incomplete and inaccurate recordkeeping.
- The Board placed the nursing assistant certificate of Eric T. Sheppard (200312126CNA) on probation for two years, for using intoxicants to the extent that he is unable to perform CNA duties safely. Mr. Sheppard resides in Phoenix, Ore.
Lane County
- The Board revoked the Registered Nurse license of Nancy P. Brooks (200141969RN) of Eugene, Ore., for gross negligence and incompetence.
- The Board suspended the nursing assistant certificate of Teresa L. McCabe (000036501CNA) of Springfield, Ore., for failing to cooperate with the Board during an investigation.
- The Board suspended the nursing assistant certificate of Hermelinda Sprague (200710468CNA) of Eugene, Ore., for failing to cooperate with the Board during an investigation.
- The Board placed the nursing assistant certificate of applicant Morgan Adrianne Bogart onprobation for two years for using intoxicants to the extent that she is unable to perform CNA duties safely. Ms. Bogart resides in Springfield, Ore.
- The Board suspended the nursing assistant certificate of Bobbie Jo J. Duran (200410178CNA) for 60 days for a Theft conviction. She resides in Eugene, Ore.
- The Board accepted the voluntary surrender of Nicole D. Shutt’s nursing assistant certificate (200110203CNA) for exploiting the client relationship for personal gain. She resides in Springfield, Ore.
Lincoln County
- The Board suspended the nursing assistant certificate of Theresa K. Kallio (200511792CNA) of Lincoln City, Ore., for failing to cooperate with the Board during an investigation.
Malheur County
- The Board accepted the voluntary surrender of Charlene J. Armstrong’s nursing assistant certificate (200010511CNA) for leaving a nursing assistant assignment without notifying her supervisor, and failing to communicate client status information to her supervisor or other appropriate personnel. She resides in Ontario, Ore.
Marion County
- The Board accepted the voluntary surrender of Sharon L. Wilson’s Practical Nurse license (089003300LPN) for failing to act to preserve client safety and failing to communicate client status to members of the health care team. She resides in Salem, Ore.
Multnomah County
- The Board suspended the nursing assistant certificate of Reola Box (000020875CNA) of Portland, Ore., for violating the terms of a Board Order and failing to cooperate with the Board during an investigation.
- The Board reprimanded the Registered Nurse license of Mariacecilia E. Susi (093003085RN) of Portland, Ore., falsifying a client record, incomplete recordkeeping, and failing to act to preserve client safety.
- The Board reprimanded the nursing assistant certificate of Rodney Wells Cole (200410369CNA) of Portland, Ore., for lying on his CNA renewal application.
- The Board reprimanded the Registered Nurse license of Julie C. Burns (087000050RN) of Portland, Ore., for leaving her nursing assignment without notifying the appropriate personnel.
- The Board reprimanded the Practical Nurse license of William J. Lane (200530226LPN) of Portland, Ore., for client neglect and incomplete recordkeeping.
- The Board suspended the Registered Nurse license of Carey W. Cullen (091006394RN) of Portland, Ore., for 60 days for conduct derogatory to the standards of nursing.
- The Board accepted the voluntary surrender of Mary E. Murphy’s Registered Nurse license (087003246RN) for using intoxicants to the extent she is unable to practice nursing safely. She resides in Portland, Ore.
- The Board suspended Suzanne K. Ruddell’s Registered Nurse license (200240120RN) for 90 days, followed by probation for two years, for failing to communicate information regarding a client’s status to members of the health care team and failing to conform to the essential standards of nursing practice. She resides in Gresham, Ore.
Wallowa County
- The Board accepted the voluntary surrender of Tanya Jo Gorbett’s nursing assistant certificate (200112073CNA) for using a controlled substance to the extent she is unable to perform CNA duties safely. She resides in Enterprise, Ore.
Washington County
- The Board denied the nursing assistant application of Isis S. Zeron of Beaverton, Ore., for Identity Theft and Forgery convictions and for lying on her application.
Yamhill County
- The Board revoked the nursing assistant certificate of Sara R. Hunt (200311758) of Newberg, Ore., for abusing a client and removing drugs from the workplace without authorization.

- Washington—The Board accepted the voluntary surrender of David J. Lydford’s Registered Nurse license (077039607RN) for verbally abusing a client. He resides in Lakewood, Wash.
- Washington—The Board suspended Timothy Charters’ Practical Nurse license (200630103LPN) for 30 days for failing to cooperate with the Board during an investigation. He resides in White Salmon, Wash.
- Texas—The Board accepted the voluntary surrender of Cynthia L. Shortle’s nurse practitioner certificate (200550171NP) and Registered Nurse license (200542631RN) for having a mental condition that prevents her from practicing safely. She resides in San Antonio, Texas.
