The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 18 nurses or nursing assistants during its March 13, 2008, Board Meeting.
Coos County
- The Board accepted the voluntary surrender of Linda L. Egbert’s practical nurse license (00004995LPN) for violating the terms and conditions of a Board order. Ms. Egbert resides in Coos Bay, Ore.
Lane County
- The Board revoked the nursing assistant certificate of Melisa A. J. Jaragosky (200511659CNA) of Springfield, Ore., for an Identity Theft conviction.
- The Board revoked the Registered Nurse license of Marie A. Kilroy (200641024RN) of Springfield, Ore., for failing to comply with the terms and conditions of the Nurse Monitoring Program and practicing nursing when impaired.
- The Board placed the Registered Nurse license of applicant Haskel Paul Stone, III, on probation for two years for a conviction for Conspiracy to Commit Securities Fraud. Mr. Stone resides in Eugene, Ore.
Linn County
- The Board reprimanded the practical nurse license of Erin J. Harlacher (200530025LPN) of Albany, Ore., for inaccurate and incomplete recordkeeping, assigning persons to perform functions beyond their scope, and failing to conform to the essential standards of acceptable nursing practice.
Marion County
- The Board suspended the nursing assistant certificate of Cindy L. Davidson (200412205CNA) of Salem, Ore., for failing to cooperate with the Board during an investigation.
- The Board suspended the practical nurse license of Karen L. Metz (093003166LPN) for a minimum of 60 days with conditions for falsifying a client record, failing to answer questions truthfully and failing to conform to the essential standards of acceptable nursing practice. Ms. Metz resides in Salem, Ore.
Multnomah County
- The Board voted for an emergency suspension of Laura E. Cassily’s Registered Nurse license (094000332RN) for convictions for DUII and Reckless Burning, practicing nursing while impaired, failure to answer questions truthfully, and practicing when unable due to a psychological impairment. Ms. Cassily resides in Portland, Ore.
Polk County
- The Board suspended the nursing assistant certificate of Nadeyda C. Caruso (000015375CNA) of Dallas, Ore., for failing to cooperate with the Board during an investigation.
- The Board accepted the voluntary surrender of Denise A. Hawks’ practical nurse license (087003314LPN) for a physical condition that prevents her from practicing nursing safely. Ms. Hawks resides in Salem, Ore.
Washington County
- The Board placed the practical nurse license of Divinity D. Arciaga-Tanke (200630407LPN) on 60-day suspension with conditions, followed by probation for two years for client neglect and failing to conform to the essential standards of acceptable nursing practice. Ms. Arciaga-Tanke resides in Hillsboro, Ore.
- The Board accepted the voluntary surrender of Carrie L. Deatley’s nursing assistant certificate (000020084CNA) for failing to meet the work hours requirement for certification. Ms. Deatley resides in Beaverton, Ore.
- The Board accepted the voluntary surrender of Joseph Beau E. Donaldson’s nursing assistant certificate (200512139CNA) for client abuse. Mr. Donaldson resides in Beaverton, Ore.
- The Board revoked the nursing assistant certificate of Glenda D. Ramirez (200313287CNA) of Hillsboro, Ore., for client abuse, neglect and violating the terms and conditions of a Board order.
- The Board accepted the voluntary surrender of Joyce E. Pivetti’s Registered Nurse license (090006574RN) for a conviction for Theft. She resides in Beaverton, Ore.
Yamhill County
- The Board accepted the voluntary surrender of Beth A. Roessel’s Registered Nurse license (086000045RN) for practicing nursing while impaired. Ms. Roessel resides in Newberg, Ore.
Out of State
- Washington—The Board reprimanded the practical nurse license of Lana M. Leveque (096005185LPN) of Vancouver, Wash., for practicing nursing when impaired.
- California—The Board accepted the voluntary surrender of Angela Kim’s Registered Nurse license (095006503RN) for convictions for Manslaughter and Conspiracy to Commit Aggravated Murder. Ms. Kim resides in Manton, Calif.
