The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 62 nurses or nursing assistants during its June 17, 2009 Board Meeting.
Clackamas County
- The Board reprimanded the nursing assistant certificate of applicant Sasha Kelsay of Portland, Ore., for failing to answer questions truthfully during the certification process.
- The Board reprimanded the nurse practitioner certificate of Monica Ferguson (200650008NP) of Happy Valley, Ore., with conditions, for improperly delegating tasks of nursing care.
- The Board reprimanded the Registered Nurse license of Julie A. Reyes (200341625RN) of Lake Oswego, Ore., with conditions for leaving a nursing assignment without notifying appropriate personnel, incomplete recordkeeping and failing to communicate client status information to members of the health care team.
- The Board placed the nursing assistant certificate of Skyler M. Swan (200612388CNA) of Oregon City, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
- The Board suspended the Registered Nurse license of Cheryl L. Cruz (200541673RN), of West Linn, Ore., for 14 days for violating the terms and conditions of a Board Order.
- The Board issued an Emergency Suspension of Lynette E. Rodd’s Registered Nurse license (089007266RN) for practicing nursing while impaired, failure to provide requested documents to the Board, and failure to cooperate with the Board during an investigation. She resides in Welches, Ore.
Clatsop County
- The Board placed the nursing assistant certificate of Gary A. Rogers (000028920CNA) of Seaside, Ore., on probation for two years for using intoxicants to the extent he was unable to perform CNA duties safely.
- The Board revoked the Registered Nurse license of Christine L. Haley (200341786RN) of Astoria, Ore., for using intoxicants to the extent she was unable to practice nursing safely.
Coos County
- The Board accepted the voluntary surrender of Brandy J. Haga’s nursing assistant certificate (200810191CNA) for violating the terms and conditions of a Board Order. She resides in Coos Bay, Ore.
Curry County
- The Board placed the nursing assistant certificate of Maryssa N. Pansevicius (200712291CNA) of Brookings, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
- The Board suspended the Practical Nurse license of Diane E. Bailey (200830104LPN), of Brookings, Ore., indefinitely for failure to cooperate with the Board during an investigation.
Deschutes County
- The Board accepted the voluntary withdrawal of Duane R. Little’s CNA application for misrepresentation during the certification process, using an intoxicant to the extent he was unable to perform CNA duties safely, and failure to answer questions truthfully. He resides in Bend, Ore.
- The Board accepted the voluntary surrender of Becky A. Lusk’s Registered Nurse license (097003277RN) for failing to comply with the terms and conditions of the Nurse Monitoring Program. She resides in Bend, Ore.
Douglas County
- The Board suspended the nursing assistant certificate of Dusty L. Eagles (200612311CNA), of Gardiner, Ore., indefinitely for failure to cooperate with the Board during an investigation.
- The Board suspended the nursing assistant certificate of Rachel L. Everhart (200112108CNA), of Roseburg, Ore., indefinitely for failure to cooperate with the Board during an investigation.
- The Board placed the nursing assistant certificate of Paul W. Young (000026871CNA) of Roseburg, Ore., on probation for two years for using intoxicants to the extent he was unable to perform CNA duties safely.
- The Board suspended the nursing assistant certificate of Valarie R. Silva-Horta (200712371CNA), of Umpqua, Ore., for 60-days with conditions, for failing to respect client dignity and client neglect.
Jackson County
- The Board imposed a $1,000 civil penalty against Sheri Engler, of Medford, Ore., a nurse imposter.
Josephine County
- The Board suspended the nursing assistant certificate of Susan A. Wisniewski (200810771CNA), of Grants Pass, Ore., indefinitely for failure to cooperate with the Board during an investigation.
- The Board reprimanded the Registered Nurse license of Patricia A. Eaton (090000272RN) of Grants Pass, Ore., with conditions, for failing to respect client dignity and failing to conform to the essential standards of acceptable nursing practice.
- The Board accepted the voluntary surrender of Susan R. Fahey’s Registered Nurse license (097000522RN) for violating the terms and conditions of a Board Order. She resides in Grants Pass, Ore.
Klamath County
- The Board revoked the nursing assistant certificate of Deirdre L. Solyst-Matthies (200512625CNA) of Klamath Falls, Ore., for using the client relationship to exploit the client for personal gain.
Lane County
- The Board suspended the nursing assistant certificate of Margaret T. Reyes (200212709CNA), of Eugene, Ore., indefinitely for failure to cooperate with the Board during an investigation.
- The Board suspended the Registered Nurse license of Sharon L. Carter (087000125RN), of Florence, Ore., for 120 days with conditions for implementing standards of nursing practice which jeopardized patient safety, inaccurate recordkeeping, falsifying a client record, and failing to communicate information regarding client status to other individuals.
- The Board accepted the voluntary surrender of Riley K. Doughan’s nursing assistant certificate (200510360CNA) for violating the terms and conditions of a Board Order. She resides in Eugene, Ore.
Lincoln County
- The Board accepted the voluntary surrender of Cindy L. Yakes’ medication aide certificate (200720123CMA) for inaccurate and incomplete recordkeeping, failing to administer medications as ordered and altering the medication administration record. She resides in Waldport, Ore.
- The Board accepted the voluntary surrender of Kelly M. Rudisill’s medication aide certificate (200420092CMA) for inaccurate and incomplete recordkeeping, failing to administer medications as ordered and altering the medication administration record. She resides in Toledo, Ore.
- The Board placed the Registered Nurse license of Christine E. Klecker (000028245RN) of Lincoln City, Ore., on probation for two years for implementing standards of practice which jeopardize patient safety, inaccurate recordkeeping, and failing to conform to the essential standards of acceptable nursing practice.
Malheur County
- The Board reprimanded the Registered Nurse license of Nichol A. King (200241568RN) of Vale, Ore., with conditions, for implementing standards of nursing care which jeopardize patient safety.
Marion County
- The Board revoked the nursing assistant certificate of Starr D. Anderson (200011070CNA) of Gervais, Ore., for using a controlled substance to the extent she was unable to perform CNA duties safely, inaccurate recordkeeping, falsifying agency records, and client neglect.
- The Board revoked the Practical Nurse license of Douglas W. Bunselmeyer (200730154LPN) of Salem, Ore., for failure to comply with the terms and conditions of the Nurse Monitoring Program.
- The Board reprimanded the nursing assistant certificate of Chad J. Skinner (200911685CNA) of Salem, Ore., for failing to answer questions truthfully.

- The Board reprimanded the Registered Nurse license of Evan R. Penfield (200541062RN) of Portland, Ore., for failing to answer questions truthfully during the licensure process.
- The Board reprimanded the nursing assistant certificate of Kelly J. Crace (200313226CNA) of Portland, Ore., with conditions, for performing acts beyond her authorized duties.
- The Board suspended the Registered Nurse license of Cathy M. Howard (081001331RN), of Portland, Ore., for three years due to a mental condition that prevents her from practicing nursing safely.
- The Board suspended the Registered Nurse license of Jannette L. Rickard (096000140RN), of Gresham, Ore., for at least 90 days, to be followed by probation for two years, for aiding an individual to violate any law, rule or regulation intended to guide the conduct of nurses or other health care providers, failing to communicate information regarding the client’s status, failing to conform to the essential standards of acceptable nursing practice, and violating the terms and conditions of a Board Order.
- The Board accepted the voluntary surrender of Ruthann M. Brown’s nursing assistant certificate (200710327CNA) for violating the terms and conditions of a Board Order. She resides in Portland, Ore.
Polk County
- The Board suspended the Registered Nurse license of Lyn M. Walters (095007055RN), of Dallas, Ore., for 30-days, to be followed by two-years probation, for failing to respect client rights, inaccurate recordkeeping, falsifying a client record, and failing to answer questions truthfully.
Tillamook County
- The Board reprimanded the Registered Nurse license of Valerie D. Olson (091003397RN) of Tillamook, Ore., for inaccurate recordkeeping, modifying standards of practice which jeopardize patient safety, and failing to conform to the essential standards of acceptable nursing practice.
Umatilla County
- The Board reprimanded the nursing assistant certificate of Kari B. Purchase (200612524CNA) of Pendleton, Ore., for performing acts beyond her authorized scope of duties.
Union County
- The Board accepted the voluntary surrender of Melissa E. Coe’s Registered Nurse license (096007197RN) for failing to comply with the terms and conditions of the Nurse Monitoring Program. She resides in Elgin, Ore.
Wallowa County
- The Board placed the nursing assistant certificate of Leslie J. Todd (200611911CNA) of Beaverton, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
Washington County
- The Board accepted the voluntary surrender of Mali J. Knapp’s Registered Nurse license (200040480RN) for violating the terms and conditions of a Board Order. She resides in Portland, Ore.

- The Board placed the nursing assistant certificate of Chrystal M. Blankenship (200411735CNA) of McMinnville, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
- The Board accepted the voluntary surrender of Roberta Vandyke’s Registered Nurse license and Women’s Health Care Nurse Practitioner certificate (000022302RN/000022302N7) for incomplete recordkeeping and prescribing in an unsafe manner according to acceptable and prevailing standards. - California—The Board suspended the nursing assistant certificate of John I. Castro (200812404CNA), of Sonora, Calif., indefinitely for failure to cooperate with the Board during an investigation.
- Louisiana—The Board revoked the Registered Nurse license of Sharon A. Fontenot (200742950RN) of New Iberia, La., for failing to cooperate with the Board during an investigation.
- Kansas—The Board reprimanded the Registered Nurse license of Lee A. French (200041087RN) of Lawrence, Kan., for falsifying a client record and failing to conform to the essential standards of acceptable nursing practice.
