The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 37 nurses or nursing assistants during its June 11, 2008, Board Meeting.
Baker County
- The Board placed the nursing assistant certificate of Margaret G. McCall-Edvalson (200511281CNA) on probation for two years for using intoxicants to the extent that she was unable to perform CNA duties safely. She resides in Baker City, Ore.
Benton County
- The Board reprimanded the Registered Nurse license of Debra Lynne (200642684RN) of Corvallis, Ore., for incomplete recordkeeping and failing to conform to the essential standards of acceptable practice.
Clackamas County
- The Board revoked the nursing assistant certificate of Carmen St. Clare Ray (200310649CNA) of Milwaukie, Ore., for removing drugs from the workplace without authorization and failing to cooperate with the Board during an investigation.
- The Board accepted the Voluntary Surrender of Leanna Nolan’s Practical Nurse license (083043023LPN) for failing to comply with the terms and conditions of the Nurse Monitoring Program. She resides in West Linn, Ore.
- The Board accepted the Voluntary Surrender of Erna E. Lewis’ Registered Nurse license (000033031RN) for failing to modify the plan of care and failing to conform to the essential standards of acceptable nursing practice. She resides in Oregon City, Ore.
Columbia County
- The Board denied the nursing assistant application of Sheila A. Caso-Murray of Rainier, Ore., for failing to respect client dignity and rights, and failing to cooperate with the Board during the course of an investigation.
- The Board reprimanded the nursing assistant certificate of Sabrina L. Vertrees (200610821CNA/200820038CMA) of Saint Helens, Ore., for failing to competently perform CNA duties.
Coos County
- The Board suspended the nursing assistant certificate of Heather M. Maine (200711295CNA) of Coos Bay, Ore., for failing to cooperate with the Board during an investigation.
Deschutes County
- The Board placed the nursing assistant certificate of Pam K. Moulton (000034274CNA) on probation for two years for using intoxicants to the extent that she was unable to perform CNA duties safely. She resides in Redmond, Ore.
Douglas County
- The Board reprimanded the Registered Nurse license of Brian J. Rogers (088003275RN) of Oakland, Ore., for performing acts beyond authorized scope, falsifying an agency record, and failing to conform to the essential standards of acceptable nursing practice.
- The Board reprimanded the Family Nurse Practitioner certificate of Judith D. Stensland (200242613RN/200250162NP) of Roseburg, Ore., for failing to take action to promote client safety, failure to properly assess and document client assessments when prescribing drugs, and failing to conform to the essential standards of acceptable nursing practice.
Jackson County
- The Board suspended the Practical Nurse license of Lauri K. Wallace (092005260LPN) of Shady Cove, Ore., for failing to cooperate with the Board during an investigation.
- The Board accepted the Voluntary Surrender of Susan L. Clark’s Registered Nurse license (088006023RN) for failing to comply with the terms and conditions of the Nurse Monitoring Program, and failing to conform to the essential standards of acceptable nursing practice. She resides in Central Point, Ore.
- The Board accepted the Voluntary Surrender of Heidi S. Haney’s Family Nurse Practitioner and Registered Nurse licenses (094006774RN/200650015NP) for failing to comply with the terms and conditions of the Nurse Monitoring Program, incomplete recordkeeping and failing to properly document client assessment when prescribing. Ms. Haney resides in Central Point, Ore.
Josephine County
- The Board reprimanded the nursing assistant certificate of Dale B. Houle (200312770CNA) of Grants Pass, Ore., for failing to answer questions on a certification application truthfully.
- The Board suspended the nursing assistant certificate of Troy L. Winger (200710068CNA) of Grants Pass, Ore., for failing to cooperate with the Board during an investigation.
- The Board reprimanded the nursing assistant certificate of Maryanna Burns (200011385CNA) of Selma, Ore., for leaving an assignment without properly notifying a supervisor.
- The Board reprimanded the Family Nurse Practitioner certificate of Denise H. Duren (087006741RN/087006741N1) of Grants Pass, Ore., for failing to report actual or suspected incidents of client abuse through proper channels.
Lane County
- The Board reprimanded the Registered Nurse license of Daphne L. Watson (088003252RN) of Walterville, Ore., for failing to take action to promote client safety and failing to follow through with the plan of care.
Marion County
- The Board reprimanded the Practical Nurse license of Dawn A. Adams (097003227LPN) of Keizer, Ore., due to a Theft conviction and failing to answer questions truthfully.
- The Board suspended the Registered Nurse license of Kerry E. Self (092003135RN) of Silverton, Ore., for 30 days for failing to take action to preserve client safety, client abuse and modifying standards of nursing practice which jeopardize patient safety.
Multnomah County
- The Board accepted the Voluntary Surrender of Jodyne M. Haugen’s Registered Nurse license (085080877RN) due to convictions for sexual abuse, attempted unlawful sexual penetration and using a child in a display of sexually explicit conduct. Ms. Haugen resides in Portland, Ore.
- The Board suspended the Practical Nurse license of Tiffany A. Dixon (200730350LPN) of Portland, Ore., for failing to cooperate with the Board during an investigation.
- The Board denied the Family Nurse Practitioner application of Marlene K. Peters (200340746RN) of Portland, Ore., for not meeting the clinical practicum requirement.
- The Board reprimanded the Registered Nurse license of Cathy M. Howard (081001331RN) of Portland, Ore., for violating privacy rights and unauthorized removal of client records from the workplace.
- The Board reprimanded the Registered Nurse license of Stephen E. Tait (088003165RN) of Portland, Ore., for failing to report actual or suspected incidents of client abuse through the proper channels and failing to conform to the essential standards of acceptable nursing practice.
- The Board suspended the nursing assistant and medication aide certificates of Erlinda E. Quintana (000016563CNA/000016563CMA) for 60 days for falsifying a client or agency record, inaccurate recordkeeping and failing to document medications as administered. She resides in Portland, Ore.
- The Board suspended the Registered Nurse license of Suzanne G. Franklin (200141129RN) for a minimum of 30 days, to be followed by probation for two years, for using the client relationship for personal gain, falsifying a client record, and failing to answer questions truthfully. She resides in Portland, Ore.
Polk County
- The Board imposed a $200 civil penalty against Bonnie Woods’ Registered Nurse license (076037134RN) for practicing without a current license. Ms. Woods resides in Dallas, Ore.
Umatilla County
- The Board imposed a $325 civil penalty against Susan M. Shepherd’s Registered Nurse license (099007106RN) for practicing without a current license. Ms. Shepherd resides in Pendleton, Ore.
Washington County
- The Board placed the Registered Nurse license of Betty J. Richmond (000007806RN) on probation for 36 months, following successful completion of a re-entry program, for conduct related to impaired function.
- The Board denied the Registered Nurse renewal application of Terry M. Hjorten (090000675RN) of Beaverton, Ore., for a mental condition that prevents him from practicing safely.
- The Board reprimanded the Registered Nurse license of Michelle L. Mattraw (081047039RN) of Tigard, Ore., for leaving a client care assignment without notifying a supervisor.
- The Board reprimanded the Psychiatric/Mental Health Nurse Practitioner certificate and Registered Nurse license of Peggy S. Casebeer (078033394RN/200250147NP) of Beaverton, Ore., for incomplete recordkeeping, and failing to conform to the essential standards of acceptable nursing practice.
- The Board reprimanded the Registered Nurse license of Olivia M. Wahl (200540612RN) of Cornelius, Ore., for failing to comply with the terms and conditions of the Nurse Monitoring Program and failing to conform to the essential standards of acceptable nursing practice.
- The Board placed the nursing assistant and medication aide certificates of Angelica M. Spurger (000020179CNA/000020179CMA) on probation for two years for using intoxicants to the extent that she was unable to perform CNA duties safely. She resides in Portland, Ore.
Out of State
- Colorado— The Board suspended the Registered Nurse license of Barbara A. Gentner (096006895RN) of Loveland, Colo., for failing to cooperate with the Board during an investigation.
