The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 40 nurses or nursing assistants during its January 8, 2009, Board Meeting.
Benton County
- The Board suspended the nursing assistant certificate of Shelly L. Lee (200512853CNA), of Corvallis, Ore., indefinitely for failure to cooperate with the Board during an investigation.
- The Board suspended the nursing assistant certificate of Robert J. Stanturf (000029428CNA), of Corvallis, Ore., indefinitely for failure to cooperate with the Board during an investigation.
Clackamas County
- The Board revoked the nursing assistant certificate of Danielle Bauer (200510507CNA) of Molalla, Ore., for misrepresentation during the licensure process and violating the terms and conditions of a Board Order.
Clatsop County
- The Board placed the nursing assistant certificate of Misty H. McBride (000036736CNA) of Seaside, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
Coos County
- The Board accepted the Voluntary Surrender of Sheila A. Lortie’s Registered Nurse license (200242325RN) for a physical condition that prevents safe nursing practice. She resides in Coos Bay, Ore.
- The Board accepted the Voluntary Surrender of Dawn A. Murphy’s nursing assistant certificate (200313659CNA) due to an Identity Theft conviction. She resides in Coos Bay, Ore.
Deschutes County
- The Board revoked the nursing assistant certificate of Nancy J. Koehler (200512381CNA) of Redmond, Ore., for violating the terms and conditions of a Board Order.
- The Board accepted the Voluntary Surrender of Pam K. Moulton’s nursing assistant certificate (000034274CNA) for violating the terms and conditions of a Board Order. She resides in Redmond, Ore.
Douglas County
- The Board suspended the nursing assistant certificate of Kevin D. Pilgram (200712193CNA), of Glendale, Ore., indefinitely for failure to cooperate with the Board during an investigation.
- The Board placed the nursing assistant certificate of Jennifer M. Espinoza (200910091CNA) of Reedsport, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.

- The Board revoked the nursing assistant certificate of Eric T. Sheppard (200312126CNA) of Phoenix, Ore., for violating the terms and conditions of a Board Order.
- The Board placed the nursing assistant and medication aide certificates of Patricia A. Pierce (000035860CNA/200220040CMA) of Medford, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
- The Board revoked the nursing assistant certificate of Sarah K. Tobiasson (200712032CNA) of Phoenix, Ore., for violating the terms and conditions of a Board Order.
Josephine County
- The Board suspended the nursing assistant certificate of Joshua J. Eldred (200710411CNA), of Grants Pass, Ore., indefinitely for failure to cooperate with the Board during an investigation.
Lane County
- The Board revoked the Registered Nurse license of Meghan C. Lee (200642069RN) of Eugene, Ore., for failure to comply with the terms and conditions of the Nurse Monitoring Program.
- The Board revoked the Registered Nurse license of Debra A. Miller (096006495RN) of Junction City, Ore., for using the nurse-client relationship to exploit the client, failing to take action to preserve client safety, and failure to conform to the essential standards of acceptable practice.
- The Board accepted the Voluntary Surrender of Judith L. Cutting’s Registered Nurse license (086006441RN) for failure to provide requested documents, and failure to notify the Board in writing of education program closure date. She resides in Cottage Grove, Ore.
- The Board accepted the Voluntary Surrender of Greg S. Glennon’s Practical Nurse license (200230200LPN) for unauthorized removal of narcotics from the workplace, failure to answer questions truthfully and practicing nursing while impaired. He resides in Eugene, Ore.
Lincoln County
- The Board accepted the Voluntary Surrender of Betty F. Schaak’s nursing assistant certificate (000023726CNA) for willful misrepresentation during the licensure process, client neglect, client abuse, and failure to answer questions truthfully. She resides in Lincoln City, Ore.
Linn County
- The Board revoked the Practical Nurse license of Erin J. Harlacher (200530025LPN) of Albany, Ore., for violating the terms and conditions of a Board Order.
- The Board placed the nursing assistant certificate of Ruthann M. Brown (200710327CNA) of Albany, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.

- The Board denied the nursing assistant renewal application of Kristine A. Aldous (200413048CNA) of Salem, Ore., for conduct related to impaired function.
- The Board revoked the Practical Nurse license of Nora E. - The Board revoked the nursing assistant certificate of Rebecca A. Nelson (000038204CNA) of Portland, Ore., for a Criminal Mistreatment conviction and failing to answer questions truthfully.
- The Board placed the nursing assistant certificate of Debra L. King (000039422CNA) of Beaverton, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely, misrepresentation during the renewal process, and failure to answer questions truthfully.
- The Board accepted the Voluntary Surrender of Scott W. Smith’s Registered Nurse license (096007829RN) due to revocation of his Texas RN license. He resides in Beaverton, Ore.
- The Board revoked the nursing assistant certificate of Deborah D. Wilson (200411095CNA) of Beaverton, Ore., due to a second-degree Assault conviction and a mental condition that prevents her from safely performing CNA duties.
Yamhill County
- The Board suspended the nursing assistant and medication aide certificates of Jennifer M. Troutman (000038361CNA/200420103CMA), of Dayton, Ore., indefinitely for failure to cooperate with the Board during an investigation.
