The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 48 nurses or nursing assistants during its February 13, 2008, Board Meeting.
Clackamas County
- The Board reprimanded the nursing assistant certificate of Amanda L. Morris (200410036CNA) of Happy Valley, Ore., for performing acts beyond her authorized duties.
- The Board reprimanded the Registered Nurse license of Mindy Lee Stewart (200241442RN) of West Linn, Ore., for incomplete recordkeeping and failing to conform to the essential standards of acceptable nursing practice.
- The Board placed the nursing assistant certificate of Sherry D. Taylor (200211385CNA) on probation for two years for failing to answer licensure questions truthfully and for using intoxicants to the extent that she is unable to perform CNA duties safely. Ms. Taylor resides in Oregon City, Ore.
- The Board placed the nursing assistant certificate of Antony M. Cusma (200512405CNA) on probation for two years for using intoxicants to the extent that he is unable to perform CNA duties safely. Mr. Cusma resides in Milwaukie, Ore.
- The Board accepted the voluntary surrender of Rachel C. Varnum’s Registered Nurse license (081046781RN) and nurse practitioner certificate (081046781N6) for abuse of prescriptive authority, failing to take action to preserve client safety, and using a controlled substance to the extent that she is unable to practice nursing safely. She resides in Lake Oswego, Ore.
Coos County
- The Board denied the Registered Nurse license application of Jeffery R. Inks, of North Bend, Ore., because of nursing license suspensions in Pennsylvania and South Carolina.
Crook County
- The Board placed the Practical Nurse license of Louis Kai Kimes (200330031LPN) on probation for two years, for using intoxicants to the extent that he is unable to practice nursing safely. Mr. Kimes resides in Prineville, Ore.
Deschutes County
- The Board reprimanded the Registered Nurse license of Julia L. Truelove (097006775RN) of Redmond, Ore., for performing acts beyond her authorized scope-of-practice.
Douglas County
- The Board placed the nursing assistant certificate of Wayne H. Jones (200510788CNA) on probation for one year, for leaving a nursing assistant assignment without properly notifying his supervisor, and having a mental condition that impedes his ability to perform CNA duties safely. Mr. Jones resides in Roseburg, Ore.
- The Board placed the nursing assistant certificate of Camille L.K. Thompson (200612019CNA) on probation for two years for using intoxicants to the extent that she is unable to perform CNA duties safely. Ms. Thompson resides in Roseburg, Ore.
Jackson County
- The Board suspended the nursing assistant certificate of Catherine M. Thomson-Smith (200511920CNA) of Medford, Ore., for failing to cooperate with the Board during an investigation.
- The Board accepted the Voluntary Surrender of Christina M. Wright’s Practical Nurse license (200530260LPN) for failing to conform to the essential standards of acceptable nursing practice. Ms. Wright resides in Talent, Ore.
- The Board reprimanded the Registered Nurse license of William P. Daugherty (088003261RN) of Central Point, Ore., for incomplete recordkeeping, failing to communicate client information to the health care team, and for performing acts beyond his authorized scope-of-practice.
- The Board accepted the Voluntary Surrender of Shelly S. Byrd’s Registered Nurse license (093006076RN) for unauthorized removal of narcotics from the workplace, failing to modify the plan of care based on nursing assessment and failing to conform to the essential standards of acceptable nursing practice. Ms. Byrd resides in Trail, Ore
Josephine County
- The Board suspended the nursing assistant certificate of Megan M. Kelly (200212916CNA) of Grants Pass, Ore., for failing to cooperate with the Board during an investigation.
Klamath County
- The Board placed the Registered Nurse license of Vicki Lou Jack (200141938RN) on probation for two years, for inaccurate recordkeeping and failing to conform to the essential standards of acceptable nursing practice. Ms. Jack resides in Klamath Falls, Ore.
Lane County
- The Board suspended the nursing assistant certificate of Jessica K. Judd (200312587CNA) of Eugene, Ore., for failing to cooperate with the Board during an investigation.
- The Board suspended the nursing assistant certificate of Amy M. Kascak (200310934CNA) of Eugene, Ore., for failing to cooperate with the Board during an investigation.
- The Board suspended the nursing assistant certificate of Melisa A. Jaragosky (200511659CNA) of Springfield, Ore., for failing to cooperate with the Board during an investigation.
- The Board reprimanded the nursing assistant certificate of Tonya L. Claxton (000025254CNA) of Eugene, Ore., for leaving a nursing assistant assignment without properly notifying her supervisor.

- The Board reprimanded the Registered Nurse license of Julie Ann Ryan (200340978RN) of Lebanon, Ore., for failing to conform to the essential standards of acceptable nursing practice.
- The Board accepted the Voluntary Surrender of Richard C. Hill’s nursing assistant certificate (000020470CNA) due to a mental condition that impedes his ability to perform CNA duties safely. Mr. Hill resides in Albany, Ore.

Umatilla County
- The Board suspended the Practical Nursing license of Amy B. Hasbrouck-Gaines (200530277LPN) of Hermiston, Ore., for failing to cooperate with the Board during an investigation.
Union County
- The Board accepted the Voluntary Surrender of Mary T. Green’s Practical Nurse license (000007371LPN) for failing to implement the client’s plan of care. Ms. Green resides in Elgin, Ore.
Wasco County
- The Board denied the nursing assistant certificate application of Jamie Leigh Nagel, of The Dalles, Ore., for falsifying her application and using controlled substances to the extent injurious to self or others.
- The Board placed the nursing assistant certificate of Dawna M. Lauzon (200810367CNA) on probation for two years for using intoxicants to the extent that she is unable to perform CNA duties safely. Ms. Lauzon resides in The Dalles, Ore.
Washington County
- The Board suspended the nursing assistant certificate of Lisa Higgs (200213088CNA) of Beaverton, Ore., for failing to cooperate with the Board during an investigation.
- The Board reprimanded the nursing assistant certificate of Sondera A. Leifermann (200212595CNA) of Beaverton, Ore., for falsifying her CNA renewal application.
- The Board reprimanded the Registered Nurse license of Leslie D. Clark (092000314RN) of Portland, Ore., for incomplete recordkeeping and leaving a nursing assignment without notifying the appropriate personnel.
- The Board placed the Registered Nurse license of Angie T. Ezelle (092000432RN) on probation for two years for using intoxicants to the extent that she is unable to practice nursing safely. Ms. Ezelle resides in Beaverton, Ore.
- The Board accepted the Voluntary Surrender of Nancy Malarchick's Registered Nurse license (096007650RN) for using a controlled substance to the extent that she is unable to practice nursing safely. Ms. Malarchick resides in Sherwood, Ore.
Yamhill County
- The Board suspended the nursing assistant certificate of Maria J. Martinez (000045264CNA) for 30 days for falsifying her CNA renewal application. Ms. Martinez resides in Dayton, Ore
Out of State
- California—The Board accepted the Voluntary Surrender of Shannon M. Stafford’s Registered Nurse license (200541257RN) for convictions for unlawfully obtaining public assistance, unlawful use of food stamps, and money laundering. Ms. Stafford resides in Carson, Calif
