The Oregon State Board of Nursing (OSBN) took disciplinary action against the following nine nurses or nursing assistants during its August 23 disciplinary Board Meeting.
Josephine County
The Board accepted the voluntary surrender of Mildred Greene’s practical nurse license (095005071LPN) for failure to conform to the essential standards of acceptable and prevailing nursing practice, and for failure to answer questions truthfully. Ms. Greene resides in Grants Pass, OR.
Lane County
The Board suspended the nursing assistant certificate of Linda C. Szymanski (000021799CNA) of Eugene, OR, for failure to cooperate with the Board during the course of an investigation.
Linn County
The Board suspended the nursing assistant certificate of Mellissa Deann Cretsinger (200510323CNA) of Albany, OR, for failure to cooperate with the Board during the course of an investigation.
Marion County
The Board suspended the nursing assistant certificate of Jessica Erin Fowler (200112160CNA) of Salem, OR, for failure to cooperate with the Board during the course of an investigation.
Multnomah County

The Board accepted the voluntary surrender of Lamin Ceesay’s nursing assistant certificate (200213136CNA) for violating the terms and conditions of a Board order and for using intoxicants to the extent that he is unable to perform the duties of a CNA safely. He resides in Troutdale, OR.
- The Board suspended the nursing assistant certificate of Valerie Michelle Lutz (000010215CNA) of Portland, OR, for failure to cooperate with the Board during the course of an investigation.
Union County
- The Board suspended the nursing assistant certificate of Susan R. Smith (200412338CNA) of La Grande, OR, for failure to cooperate with the Board during the course of an investigation.
- The Board suspended the nursing assistant certificate of Kelly A. McCray (200211929CNA) of La Grande, OR, for failure to cooperate with the Board during the course of an investigation.
Out of State
New Mexico—The Board suspended the Practical Nurse license of Jessica Ruth Weiner (200530194LPN) of Tularosa, NM, for failure to cooperate with the Board during the course of an investigation.
