Board of Nursing Disciplinary Actions Taken in April 2009
The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 72 nurses or nursing assistants during its April 22, 2009 Board Meeting.
 
Baker County
  • The Board revoked the nursing assistant certificate of Kristin N. Abbott (200812117CNA) of Baker City, Ore., for violating the terms and conditions of a Board Order.
  • The Board accepted the voluntary surrender of Kricket J. Porter’s nursing assistant certificate (200810653CNA) for violating the terms and conditions of a Board Order.  She resides in Baker City, Ore.
 
Clackamas County
  • The Board reprimanded the Registered Nurse license of Jane B. Atkin (087006576RN) of Portland, Ore., with conditions for inaccurate recordkeeping and failure to conform to the essential standards of acceptable nursing practice.
  • The Board reprimanded the nursing assistant and medication aide certificates of Heather D. Nevins (200011009CNA/200520139CMA) of Oregon City, Ore., for inaccurate and incomplete recordkeeping and failure to administer medications as ordered.
  • The Board reprimanded the nurse practitioner certificate of Barbara M. Brown (088003110N1) of Milwaukie, Ore., with conditions for prescribing drugs without adequate instructions to the client.
  • The Board reprimanded the Registered Nurse license of Jennifer L. Villanueva (096000465RN) of Milwaukie, Ore., with conditions for inaccurate and incomplete recordkeeping, performing acts beyond authorized scope, and failure to conform to the essential standards of acceptable nursing practice.
  • The Board placed the nursing assistant certificate of Theresa Cunningham (200310154CNA) of Lake Oswego, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
  • The Board suspended the nursing assistant certificate of Pamela M. Alcorn (200610370CNA), of Portland, Ore., for 30 days with conditions for client neglect and failure to competently perform CNA duties.
  • The Board suspended the nursing assistant certificate of Morgan C. Bach (200611099CNA), of Boring, Ore., for 30 days with conditions for client neglect and failure to competently perform CNA duties.
  • The Board accepted the voluntary surrender of Pamela D. Sidor’s Registered Nurse license (077038984RN) for a physical condition that prevents her from practicing safely.  She resides in West Linn, Ore.
  • The Board placed the nursing assistant certificate of applicant Bonnie M. Plumlee of Eugene, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
 
Columbia County
  • The Board placed the nursing assistant and medication aide certificates of Nicole M. Zsarko (200312368CNA/200520099CMA) of Saint Helens, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
  • The Board accepted the voluntary surrender of Krystle M. Kalbasky’s nursing assistant certificate (200710652CNA) for failure to respect client rights and dignity.  She resides in Saint Helens, Ore.
 
Coos County
  • The Board suspended the nursing assistant certificate of Kayla M. Deichler (200810534CNA), of Coos Bay, Ore., indefinitely for failure to cooperate with the Board during an investigation.
  • The Board revoked the nursing assistant certificate of Brandi K. Marshall (200710822CNA) of Coos Bay, Ore., for misrepresentation during the licensure process, and failure to answer questions truthfully.
  • The Board revoked the Registered Nurse license of Jennifer L. Walsh (097006877RN) of Coos Bay, Ore., for practicing nursing while impaired, use of intoxicants to the extent she was unable to practice safely and failure to conform to the essential standards of acceptable nursing practice.
  • The Board reprimanded the Registered Nurse license of Karyle K. Wallace (080045950RN) of North Bend, Ore., for incomplete recordkeeping and failure to conform to the essential standards of acceptable nursing practice.
  • The Board suspended the nursing assistant certificate of Tina R. Carbaugh (200610613CNA), of Coos Bay, Ore., for 30 days with conditions for altering a client record, and performing acts beyond her authorized duties.
  • The Board accepted the voluntary surrender of Bruce E. Boyd’s Practical Nurse license (200230204LPN) for failure to follow through with the plan of care, client neglect and failure to conform to the essential standards of acceptable nursing practice.  He resides in Lakeside, Ore.
  • The Board accepted the voluntary surrender of Dianne L. Elton’s Registered Nurse license (090006851RN) for violating the terms and conditions of a Board Order.  She resides in North Bend, Ore.
 
Crook County
  • The Board accepted the voluntary surrender of Rhonda K. Frakes’ Registered Nurse license (084053555RN) for failure to comply with the terms and conditions of the Nurse Monitoring Program.  She resides in Prineville, Ore.
 
Hood River County
  • The Board revoked the nursing assistant and medication aide certificates of Kathryn M. Sottile (200611683CNA/200720116CMA) of Hood River, Ore., for client abuse, failing to administer ordered medications, and falsifying the medication administration record.
  • The Board accepted the voluntary surrender of Carla L. Fies’ Registered Nurse license (099000628RN) for practicing while impaired and using intoxicants to the extent she was unable to practice nursing safely.  She resides in Hood River, Ore.
 
Jackson County
  • The Board reprimanded the nursing assistant certificate of Michelle L. Peters (000027990CNA) of Gold Hill, Ore., for failure to answer questions truthfully.
  • The Board reprimanded the nursing assistant certificate of Ryan P. Ulbricht (200712511CNA) and placed it on probation for two years for using intoxicants to the extent he was unable to perform CNA duties safely, and failure to answer questions truthfully.  He resides in of Medford, Ore.
  • The Board accepted the voluntary surrender of William F. Hampton’s Registered Nurse license (079042565RN) for practicing nursing while impaired, and inaccurate and incomplete recordkeeping.  He resides in Medford, Ore.
 
Josephine County
  • The Board revoked the nursing assistant certificate of Valerie D. Robles (000045114CNA) of Grants Pass, Ore., for violating the terms and conditions of a Board Order.
  • The Board accepted the voluntary surrender of Janet Simons’ Registered Nurse license (091000101RN) for practicing while impaired and using intoxicants to the extent she was unable to practice safely.  She resides in Grants Pass, Ore.
 
Klamath County
  • The Board suspended the Registered Nurse license of Deborah L. James (200341292RN), of Klamath Falls, Ore., indefinitely for failure to cooperate with the Board during an investigation.
 
Lane County
  • The Board denied the CNA application of Patricia A. Moon of Eugene, Ore., for Theft and Forgery convictions, and misrepresentation during the licensure process.
  • The Board suspended the Registered Nurse license of Jean M. Cannon (089007353RN), of Florence, Ore., indefinitely for failure to cooperate with the Board during an investigation.
  • The Board reprimanded the Registered Nurse license of Alan L. Freytag (094000387RN) of Springfield, Ore., for failure to answer questions truthfully.
  • The Board reprimanded the Practical Nurse license of Diana L. Siberz (200130158LPN) of Creswell, Ore., for misrepresentation during the licensure process and failure to answer questions truthfully.
  • The Board accepted the voluntary surrender of Abby L. Collver’s Practical Nurse license (095005008LPN) for failure to comply with the terms and conditions of the Nurse Monitoring Program.  She resides in Eugene, Ore.
  • The Board placed the nursing assistant certificate of Alice A. Andersen (200911059CNA) of Eugene, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
  • The Board placed the nursing assistant certificate of Michelle A. Steward (200911060CNA) of Eugene, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
 
Linn County
  • The Board suspended indefinitely the Registered Nurse license of Patricia M. Halfacre (091000140RN) of Lebanon, Ore., for failure to comply with the terms and conditions of the Nurse Monitoring Program.
 
Marion County
  • The Board revoked the nursing assistant certificate of John L. Kennedy (000019769CNA) of Salem, Ore., for engaging in sexual misconduct with a client and client abuse.
  • The Board placed the nursing assistant certificate of Debbie M. Miller (200010397CNA) of Woodburn, Ore., on probation for two years for using intoxicants to the extent that she was unable to perform CNA duties safely.
  • The Board revoked the nursing assistant certificate of Toni N. Justice (200811807CNA) of Salem, Ore., for exploiting the client relationship for personal gain.
  • The Board suspended the Practical Nurse license of Douglas W. Bunselmeyer (200730154LPN), of Salem, Ore., indefinitely for failure to cooperate with the Board during an investigation.
  • The Board suspended the Registered Nurse license of Judith A. Davis (200242598RN), of Salem, Ore., for 60 days with conditions for improperly delegating tasks of nursing care, inaccurate recordkeeping, and failing to conform to the essential standards of acceptable nursing practice.
  • The Board placed the Registered Nurse license of Kathleen K. Bell (200643070RN) of Woodburn, Ore., on probation for two years for failure to take action to preserve client safety, client neglect, inaccurate recordkeeping, and failure to conform to the essential standards of acceptable nursing practice.
 
Multnomah County
  • The Board revoked the Registered Nurse license of Brandi L. Born (099000586RN) of Gresham, Ore., for failure to comply with the terms and conditions of the Nurse Monitoring Program.
  • The Board revoked the nursing assistant certificate of Amanda Rice (000044665CNA) of Portland, Ore., for client abuse and failure to respect client rights.
  • The Board revoked the nursing assistant certificate of Jolyn M. Fenwick (000039135CNA) of Portland, Ore., for violating the terms and conditions of a Board Order.
  • The Board placed the nursing assistant certificate of Rachael F. Maddox (200911063CNA) of Portland, Ore., on probation for two years for having a mental condition that impairs her ability to perform nursing assistant duties safely.
  • The Board reprimanded the nursing assistant certificate of Nakeia L. Scott (200911051CNA) of Portland, Ore., for failure to answer questions truthfully.
  • The Board reprimanded the Registered Nurse license of Eugene W. Bush (089003303RN) of Portland, Ore., for failure to communicate information regarding client status to members of the health care team.
  • The Board placed the nursing assistant certificate of Michael Pickersgill (000024226CNA) of Portland, Ore., on probation for two years for using intoxicants to the extent he was unable to perform CNA duties safely.
  • The Board suspended the nursing assistant certificate of Angela M. Martinez (200710547CNA), of McMinnville, Ore., for 30 days for client neglect and leaving a CNA assignment without properly notifying appropriate supervisory personnel.
  • The Board suspended the Registered Nurse license of Cheanice L. Easter (099000330RN), of Portland, Ore., for 30 days with conditions for improperly delegating tasks of nursing care, inaccurate and incomplete recordkeeping, and failing to conform to the essential standards of acceptable nursing practice.
  • The Board accepted the voluntary surrender of Suzann J. Jones’ nursing assistant certificate (200712031CNA) for violating the terms and conditions of a Board Order.  She resides in Gresham, Ore.
  • The Board accepted the voluntary surrender of Kimberly T. D. Joers’ Registered Nurse license (200641699RN) for violating the terms and conditions of a Board Order.  She resides in Portland, Ore.
 
Polk County
  • The Board denied the CNA application of Shannon M. Meeker of Independence, Ore., for a Criminal Mistreatment conviction.
  • The Board accepted the voluntary surrender of Sheree V. Kumm’s nursing assistant certificate (000009933CNA) for failure to answer questions truthfully during the licensure process.  She resides in Independence, Ore.
 
Umatilla County
  • The Board accepted the voluntary surrender of Gloria D. Rodli’s Practical Nurse license (079003405LPN) for failure to take action to promote patient safety, and failure to conform to the essential standards of acceptable nursing practice.  She resides in Hermiston, Ore.
  • The Board accepted the voluntary surrender of Janice K. Dougherty’s nursing assistant certificate (000034606CNA) for performing acts beyond her authorized duties.  She resides in Pendleton, Ore.
 
Washington County
  • The Board suspended the Registered Nurse license of Dee A. Scoles (087000031RN) of Sherwood, Ore., for 60 days for engaging in sexual contact with a client and violating the rights of privacy, confidentiality or knowledge concerning the client.
  • The Board revoked the nursing assistant and medication aide certificates of Diane M. Nicholson (000030408CNA/000030408CMA) of Tualatin, Ore., for exploiting the client relationship for personal gain, failure to cooperate with the Board during an investigation, and violation of a Board Order.
  • The Board placed the nursing assistant certificate of Nicole A. Busse (200310764CNA) of Tigard, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
  • The Board suspended the Registered Nurse license of Ron Lapp (082011874RN), of Portland, Ore., for 30 days, to be followed by two years probation, for engaging in unacceptable behavior in the presence of a client, failure to respect client rights, and failure to conform to the essential standards of acceptable nursing practice.
  • The Board accepted the voluntary surrender of Lorri E. Castle’s Registered Nurse license (097000338RN) for failure to comply with the terms and conditions of the Nurse Monitoring Program.  She resides in Hillsboro, Ore.
  • The Board accepted the voluntary surrender of Deborah A. Fontaine’s Registered Nurse license (093000633RN) for unauthorized removal of narcotics from the workplace and incomplete recordkeeping.  She resides in Tualatin, Ore.
 
Yamhill County
  • The Board placed the nursing assistant and medication aide certificates of Jennifer M. Troutman (000038361CNA/200420103CMA) of Dayton, Ore., on probation for two years for using intoxicants to the extent she was unable to perform CNA duties safely.
  • The Board suspended the nursing assistant certificate of Kira L. Whiteley (200610910CNA), of McMinnvelle, Ore., for 30 days for client abuse and neglect.
  • The Board accepted the voluntary surrender of Debra L. Schwartzkopff’s Registered Nurse license (094006259RN) for a mental condition that prevents her from practicing safely.  She resides in Newberg, Ore.
 
Out of State
  • Florida—The Board accepted the voluntary surrender of Melissa R. Waller’s Registered Nurse license (200740803RN) for failure to comply with the terms and conditions of the Nurse Monitoring Program.  She resides in Tampa, Fla.
  • Indiana— The Board accepted the voluntary surrender of Carolyn C. Reuteler’s Registered Nurse license (095003031RN) for violating the terms and conditions of a Board Order.  She resides in Dillsboro, Ind.
  • Washington—The Board suspended the nursing assistant certificate of Kimberly A. Suarez (200213178CNA), of Walla Walla, Wash., indefinitely for failure to cooperate with the Board during an investigation.
  • North Carolina— The Board accepted the voluntary surrender of Ruth E. Lyons’s Registered Nurse license (077037900RN) for having been disciplined in North Carolina.  She resides in Ellenboro, N.C.
  • Hawaii— The Board accepted the voluntary surrender of Coleen F. Wilson’s Registered Nurse license (200840339RN) for falsifying an agency record and unauthorized removal of narcotics from the workplace.  She resides in Pukalani, Hawaii.
