The Oregon State Board of Nursing (OSBN) took final disciplinary action against the following 45 nurses or nursing assistants during its April 9, 2008, Board Meeting.
Baker County
- The Board placed the nursing assistant certificate of Kricket J. Porter (200810653CNA) on probation for two years for using controlled substances to the extent that she is unable to perform CNA duties safely. She resides in Baker City, Ore.
Clackamas County

Curry County
- The Board revoked the Registered Nurse license of Robert S. Spears (095006568RN) of Gold Beach, Ore., for using alcohol to the extent that he was unable to practice nursing safely.
Deschutes County
- The Board reprimanded the Registered Nurse license of Mary R. LeLouis (086006857RN) of Redmond, Ore., for a Theft conviction and failing to answer questions truthfully.
Jackson County
- The Board suspended the Registered Nurse license of James E. Mills (099000574RN) of White City, Ore., for one year with conditions for incomplete recordkeeping, failing to implement the plan of care and failing to conform to the essential standards of acceptable nursing practice.
- The Board revoked the nursing assistant certificate of Patti J. McMullen (000027141CNA) of Rogue River, Ore., for using alcohol to the extent that she is unable to perform the duties of a CNA safely.
- The Board suspended the nursing assistant certificate of Brandy R. Sewell (200612216CNA) of Medford, Ore., for 90 days for falsifying an agency record and failing to competently perform the duties of a nursing assistant.
- The Board accepted the Voluntary Surrender of Janet M. Holt’s Registered Nurse license (090000192RN) for using a controlled substance to the extent that she is unable to practice safely. She resides in Medford, Ore.
Josephine County
- The Board reprimanded the nursing assistant certificate of Cheryle L. Helton (200611043CNA) with conditions for failing to respect client rights and dignity. She resides in Wolf Creek, Ore.
Lane County
- The Board suspended the nursing assistant certificate of Jamie J. Moon (200310271CNA) of Eugene, Ore., for failing to cooperate with the Board during an investigation.
- The Board suspended the Registered Nurse license of Jocelyn R. Degroat (200041225RN) of Eugene, Ore., for failing to cooperate with the Board during an investigation.
- The Board revoked the nursing assistant certificate of Cassandra L. Ligon (200312020CNA) of Cottage Grove, Ore., for convictions for Theft and Identity Theft, and for using a controlled substance to the extent that she is unable to perform the duties of a CNA safely.
- The Board reprimanded the nursing assistant certificate of Shari L. Medart (200210052CNA) of Springfield, Ore., for performing acts beyond the authorized duties of a CNA.
- The Board reprimanded the nursing assistant certificate of Jessica K. Judd (200312587CNA) of Eugene, Ore., for functioning as a medication aide without current certification.
- The Board placed the nursing assistant certificate of Riley K. Doughan (200510360CNA) on probation for two years for using alcohol to the extent she is unable to perform CNA duties safely. She resides in Eugene, Ore.
- The Board accepted the Voluntary Surrender of Audrey C. Mathis’ nursing assistant certificate (200312769CNA) for failing to respect patient rights and dignity, client neglect and failing to competently perform CNA duties. She resides in Eugene, Ore.
- The Board accepted the Voluntary Surrender of Santos Loza Torres’ nursing assistant certificate (200610684CNA) for fraud in applying for certification. He resides in Florence, Ore.
Lincoln County
- The Board suspended the Practical Nurse license of Kari M. Burns (200330426LPN) for 60 days, with conditions, for aiding an individual to circumvent the law, and failing to conform to the essential standards of acceptable nursing practice. She resides in Toledo, Ore.

- The Board placed the nursing assistant certificate of Gifford J. Rye (000011485CNA) on probation for two years for a Harassment conviction and alcohol dependency. Mr. Rye resides in Salem, Ore.
- The Board placed the nursing assistant certificate of Heather A. Beyer (200810651CNA) on probation for two years for using a controlled substance to the extent that she is unable to perform CNA duties safely. She resides in Gates, Ore.
- The Board accepted the Voluntary Surrender of Margaret M. Johnson’s nursing assistant certificate (000038618CNA) for exploiting the client for personal gain and conduct unbecoming. She resides in Gates, Ore.
Multnomah County
- The Board reprimanded the Registered Nurse license of Rhonda T. Lee (085075228RN) of Portland, Ore., for violating patient privacy rights and confidentiality.
- The Board placed the nursing assistant certificate of CNA Applicant Debra K. McMinn on probation for two years for using a controlled substance to the extent that she is unable to perform CNA duties safely. She resides in Portland, Ore.
- The Board placed the nursing assistant certificate of Mary T. Freeman (200810652CNA) on probation for two years for using a controlled substance to the extent that she is unable to perform CNA duties safely. She resides in Portland, Ore.
- The Board suspended the Registered Nurse license of Jane M. McKay (095006412RN) for a minimum of 30 days for client neglect, inaccurate recordkeeping and failing to take action to promote client safety. Ms. McKay resides in Portland, Ore.
- The Board accepted the Voluntary Surrender of Kelsey R. Meyer’s nursing assistant certificate (200313637CNA) for a Burglary conviction and for using intoxicants to the extent she is unable to conduct CNA duties safely. She resides in Portland, Ore.
- The Board accepted the Voluntary Surrender of Laurencetta Hawkin’s Practical Nurse license (200230449LPN) for failing to take action to preserve patient safety, failing to communicate client status to the health care team and failing to conform to the essential standards of acceptable nursing practice. She resides in Portland, Ore.
- The Board revoked the nursing assistant certificate of Libra R. Gauger (000025148CNA) of Portland, Ore., for a Theft conviction and for removing drugs from the workplace without authorization.
Polk County
- The Board suspended the nursing assistant certificate of April L. Whitney (200710196CNA) of Dallas, Ore., for failing to cooperate with the Board during an investigation.
Wasco County
- The Board suspended the nursing assistant certificate of Ashlee R. Klaviter (200711659CNA) of The Dalles, Ore., for failing to cooperate with the Board during an investigation.
- The Board revoked the nursing assistant certificate of Summer A. Heebink (000034626CNA) of The Dalles, Ore., for an Assault conviction and using a controlled substance to the extent that she cannot perform CNA duties safely.
- The Board reprimanded the Registered Nurse license of Anita L. Duvall (098003009RN) with conditions for failing to follow the plan of care. Ms. Duvall resides in The Dalles, Ore.
Washington County
- The Board revoked the medication aide and nursing assistant certificates of Manzar Baradar (000028758CNA/000028758CMA) of Beaverton, Ore., for inaccurate recordkeeping, altering a client record, failing to administer medications as ordered, and failing to competently perform nursing assistant duties.
Yamhill County
- The Board placed the Registered Nurse license of Lorraine H. McLaughlin (086000509RN) on probation for two years for chemical dependency. Ms. McLaughlin resides in McMinnville, Ore.

- Tennessee—The Board revoked the Registered Nurse license of Mary P. Keene (200340783RN) of Telford, Tenn., for inaccurate recordkeeping, unauthorized removal of narcotics from the workplace, and practicing nursing while impaired by using drugs.
- Washington—The Board revoked the nursing assistant certificate of Diana E. Grant (200311048CNA) of Vancouver, Wash., for a Criminal Mistreatment conviction, client abuse, removing drugs from the workplace without authorization, and using controlled substances to the extent that she is unable to perform CNA duties safely.
- Washington—The Board placed the Registered Nurse license of Susan D. Crull (099000565RN) on probation for two years for failing to take action to preserve client safety and failing to conform to the essential standards of acceptable nursing practice. She resides in Vancouver, Wash.
- Washington—The Board placed the Registered Nurse license of Applicant Shawn M. Wager on probation for two years for a condition that makes the licensee unable to practice nursing safely. She resides in Vancouver, Wash.
- Washington—The Board suspended the Registered Nurse license (093007089RN) and Family Nurse Practitioner certificate (093007089N1) of Gayle M. Miller for one year with conditions, to be followed by one year of probation, for failing to take action to promote client safety and failing to conform to the essential standards of acceptable nursing practice. She resides in Vancouver, Wash.
