Introduction
 
The Structure Services Unit is responsible for evaluating various products to determine if they meet ODOT's specifications for use on construction projects. The results are published semi-annually in the QUALIFIED PRODUCTS LIST or "QPL." The QPL is a comprehensive listing of all finished products found to be acceptable by ODOT for use with specific categories in roadway construction and maintenance.

To Extend Conditional Period
This form is to be used when a manufacturer is nearing the end of the limited 2-year conditional approval period.

Request to Move a Product from the Conditional List to the Qualified List
This form is to be used when a manufacturer wants to have a conditionally approved product considered for inclusion on the Qualified List.

Product Name Change
This form is used by a manufacturer who wants to update our files on a change of product name, but no changes to the product itself. If there have been any changes to the product (other than the name) let us know, and submit as a new product.

Manufacturer's Name Change
This form is used by a manufacturer who wants to update our files on a change of corporate name, but no changes to the products in our system. If there have been any changes to the product let us know, and submit as a new product.
