Changing your Name

Name Change
A name change can be part of an original, renewal or replacement transaction for a driver license, driver permit or identification card. You will need to visit a DMV office in person, and will be asked to:
Proof of Full Legal Name
Your full legal name is the name as it appears on your proof of legal presence. If the name you wish to be issued under is different than your name as it appears on your proof of legal presence document, you will be required to provide at least one of the following documents verifying your legal name change.
  • An official government issued marriage certificate/license including city, county or state (signed by a government official and including a stamp to show that the document has been filed);
  • A certificate of Registered Domestic Partnership issued by city, county or state Vital Statistics agency;
  • A U.S. city, county or state court-issued divorce decree, judgment of dissolution of marriage, annulment of marriage decree, judgment of dissolution of domestic partnership, or annulment of domestic partnership;
  • A government-issued death certificate (signed by a government official and including a stamp to show that the document has been filed);
  • A U.S. city, county or state court-issued adoption decree;
  • A U.S. city, county or state court-issued legal name change decree;
  • A U.S. city, county or state court-issued custody decree or guardianship decree.
All legal name change documents must be issued by a U.S. or foreign government agency that is authorized to issue such documents. The document must be a legible and unaltered original or certified copy with a government seal, stamp or other official imprint.
 
NOTE: Changing your name on your Oregon Driver License, Permit, or ID Card does not automatically change your name on your Oregon vehicle record, title or registration. You must apply for a name change for your Oregon title within 30 days of the name change. See section below for more information.

Title & Registration
ORS 803.220 requires you to change the name on your title if your name changes by marriage or otherwise from that shown on the title. Changing your name on the title will also change your name on the registration.
 
To apply for a title in your new name you must:
  • Submit the Oregon title;
  • Submit a Request for Correction of Title Records;
  • Make a statement on the form that you are the same person, such as "Jane Smith and Jane Doe are the same person due to... (marriage, divorce, court order, etc.)." It is also helpful to include your license, permit or ID card number and date of birth;
  • Pay the title fee of $55; and
  • Take the completed form with all of the above listed items to your local DMV office or mail to:

Remember to Register to Vote
When you are issued a driver license, instruction permit, identification card or when you change your name, and are 17 years or older, you can register or re-register to vote.
