| Reporting a Problem Driver to DMV |

| At-Risk Drivers |

Family members, friends, law enforcement and social service providers may voluntarily report an unsafe driver under DMV's non-mandatory (voluntary) reporting system. The report may be based on medical condition or observed driving behaviors.
To file a voluntary report you will need to complete the Driver Evaluation Request Form (#6066), which can be downloaded or printed from this web site using your browser's Save As and Print options, or obtained from a DMV office.
DMV will not accept anonymous reports. The form requires that you supply your name, address and original signature, however, you can request that your name be kept confidential.
When filling out the form, be as thorough as possible. Describe any medical conditions that interfere with driving, as well as any unsafe driving behaviors that you have observed. If possible, recount how driving ability has declined over time or as a medical condition has progressed.
Completed Forms may be submitted by mail or at a DMV office.
DMV will review the information and may send a letter to the driver asking for a re-examination of their driving skills.

| Violent or Intoxicated Drivers |

To report drivers who are intoxicated or are exhibiting signs of "road rage," contact your local law enforcement agency by calling 911.
If you suspect the person is driving under the influence then you can call (800) 24DRUNK (or (800) 243-7865) if outside the city limits or 911 if in the city limits.
