Organ, Eye, and Tissue Donation
Overview
This page provides information on the Donor Registry and how you can become a donor.

How To Sign Up
All individuals 15 years of age or older can register as an organ, eye, and tissue donor. To sign up on the Donor Registry:
  • Ask DMV to put a “D code on your license, permit or identification card;
  • Go online at donatelifenw.org; or
  • Request a paper form by calling (800) 452-1369.
Do not rule yourself out as a donor. Criteria for donations change due to medical research, and the ability to be a donor is determined only at the time of death.
 
By signing up on the Donor Registry, you are confirming your commitment to be a donor and ensuring your decision is known and honored by your family and the organ, eye, and tissue recovery agencies.  It is extremely important you also inform your family of your wishes to be a donor.
