Please make sure we have your current clinic information. We need to have the following information: - Clinic name
- Both physical address and mailing address, (if different)
- Phone and fax numbers
- e-mail address, if available
- The names of veterinarians on your staff and
- The names of veterinarians who have left your staff (and where they went, if you know.)
If there are any changes in the above information, please call Darcie at 503-986-4689, send e-mail, or include a note with your weekly mailing of official documents.

| Distribution of copies of CVIs |

- Five part forms should be routed as follows: first two copies to State Veterinarian's Office; next two to client; final for your files.
- Four-part forms are still valid. Use them until they're gone. Send the original to ODA, next two copies to client, final copy for your files. We'll make and file a photocopy and send the original to the state of destination.
Make sure your file copy is legible. Make a photocopy of the original if necessary.

Legibility counts on official documents.
It is important to print or type both the veterinarian´s name and the full clinic name & address on all official documents: CVIs, test charts, vaccination reports, etc. If there´s a question or problem and the clinic information is incomplete and we can´t decipher the doctor´s highly stylized signature, we don´t know who to contact. A rubber stamp is OK for clinic information, but not for signatures.

Mail CVIs, test charts, and brucellosis vaccination reports to the State Veterinarian´s office on a regular schedule - weekly is preferred, but certainly no later than 15 days after issue. Send to:
