For those who have passed NPLEX BS and Clinical, the Formulary and Jurisprudence State Exams will be held on:
November 3, 2008

Location and Time:
PSOB Building, Room 445 (800 NE Oregon St., Portland OR 97202)
The Formulary is from 9:00 am - 10:30 am
The Jurisprudence is from 11:00 am - 12:00 pm

Jurisprudence & Formulary State Exam Application Deadline:
October 20, 2008

You may download application material below or you may contact the Board office for more information.

The board must have these three items before you can sit for the exams:

1. Official Graduate Transcripts
2. Graduate Diploma
3. NPLEX Scores (for all exams already taken)


Items needed for licensure:

1. Official Undergraduate Transcripts
2. Fingerprint cards

* It's a good idea to reguest your undergranduate transcripts early and to submit your fingerprint cards early
