Formulary & Jurisprudence Exam Application Deadline:

You may download application materials after December 1, 2009. Applications will not be accepted until after January 1, 2010

The board must have these three items before you can sit for the exams:

1. Official Naturopathic School Transcripts
2. Diploma from Naturopathic school (can be a copy from your original)
3. NPLEX Scores (for all exams previously taken)

As well as these items for licensure:

1. Official Undergraduate Transcripts*
2. Fingerprint cards/passing of background check*
3. Verfication from all states/jursidictions in which you have been licensed (current, expired, or otherwise)

* It's a good idea to request your undergraduate transcripts early and to submit your fingerprint cards as soon as your application has been submitted
