It is a licensee´s responsibility to advise the Board of any change of address. Oregon Law requires that licensed dentists and dental hygienists notify the Board within 30 days of any change of address. If the Board does not have your current mailing address, it is very likely that you will not receive notices regarding your license renewal. Regardless of whether or not you receive a notice of renewal, it is your responsibility to assure that your license is current. Practicing dentistry or dental hygiene in Oregon without a current license is a violation of the Dental Practice Act.
