| Licensing & Registration |
|
| All tanning devices must be registered with the State of Oregon Radiation Protection Services Tanning Bed Program, OAR 333-119-002(1). A license will be issued for each tanning device. Licenses are renewed every year on January 1. |
|
Registration cannot be transferred from one person to another person. Licenses cannot be transferred from one tanning facility to another tanning facility. Licenses cannot be transferred from one tanning device to another tanning device (OAR 333-119-0020(9)).
The registration fee is $100 for each tanning device.

| Inspections |
|
| Tanning facilities are inspected once every 2-3 years. During an inspection items are identified that result in citations for non-compliance. The most common ones are often times the easiest to avoid. One way to avoid these citations is to read the OAR 333-119 Rules for the Regulation of Tanning Devices and Facilities. The following information is designed to assist you in operating your tanning facility without making these common mistakes. |

1. Signs:
Signs are often missing due to remodeling, office rearranging, new ownership, or forgetfulness.
There are 4 signs the inspector will be looking for:
1. A copy of the Persons under age 18 (pdf) sign must be posted in public view. One sign visible at the reception area is acceptable; however, many facility owners choose to post a sign in each room, this is also acceptable.
2. A copy of the Notice to the Public (pdf) sign must be posted in public view. Many facility owners choose to post this sign in each room, this is also acceptable.
3. A copy of the Warning sign (pdf) must be posted within ONE METER (39.37 inches or approximately 3 feet) of each tanning device.
| 4. Photosensitizing agents such as prescription drugs, and some foods can affect your tanning experience. A copy of photosensitizing agents or Medication List (pdf) must be available for review by consumers (pdf). Facilities that are strictly tanning tend to post this sign in public view. Facilities that offer services in addition to tanning often post this sign in each room containing a tanning device. Either option is acceptable. |

**Note: We can provide you with copies of each of these signs; however, we understand that this is your business and our color choices might not fit your décor. Please feel free to have the signs printed on whatever color/texture paper you would like. REMEMBER the Warning Sign must uses letters at 10 mm and 5 mm in height.
2. Record Keeping: Each facility will maintain records of consumer’s exposures including date and duration of the visit (OAR 333-119-0110(1)). This information can be tracked on a computer program or by using a client card (pdf), or a combination of both. The client card will also include essential information including skin typing (see below), photosensitizing medications, pregnancy, the complete WARNING STATEMENT found in OAR 333-110-050, a signature line for clients, a signature line for parent or legal guardian if minors are tanning, and a means to record client’s complete exposure history.

Make certain all cards are filled out completely and signed. Don’t forget the cards must be filled out and signed annually (OAR 333-119-0050)! You may include other questions specific to your facility, other liability disclaimers, company logos, etc., in addition to what is on the sample card. Inspectors will be looking for these things on the client cards.

Keep track of bed maintenance! You can use the Tanning Bed/Booth Maintenance Record (pdf) form. Record all services performed on the tanning device including lamp changes, part replacement, timer tests (timer tests are required to be done annually, OAR 333-119-0100(c)(17)) etc., include the date of maintenance and the unit hours of each device.

Keep track of employee training! You can use the Log of Tanning Operator Training (pdf) form. Record all formal trainings and informal trainings on this log. Make sure copies of formal training certificates and the Log of Tanning Operator Training are available for inspector review. If you work at more than one facility have a cop y of your formal training certificate available at each facility. |

3. Facility Operations
Protective Eyewear is REQUIRED, not just recommended. Make sure every client has and uses eye protection each time they tan

Make certain the sanitizer you use is an approved sanitizer (pdf). Approved sanitizers properly disinfect without damaging acrylics. A TEST KIT is needed to insure proper concentrations in parts-per-million (ppm). Sanitizer tests are required to be done when the sanitizer is mixed initially and then weekly. |
All tanning devices must be controlled by a remote timer system located outside of the tanning room. The timer must be set by the operator.

Tanning devices and eye protection must be cleaned by a facility operator between client use, NOT by the customer!
