LIEAP Assistance For Client on the OHOP Waitlist ONLY:
For the 2006-2007 heating season, the OHOP program will be able to provide LIEAP assistance to eligible clients on the OHOP wait list. To be eligible, clients must:
1. Be on the OHOP wait list. (Note that clients may not be referred to OHOP solely for LIEAP assistance. They must be referred to OHOP based on an ongoing housing need that OHOP would assist with regardless of the client's need for LIEAP assistance)
2. Have household income at or below 60% of the statewide median family income (see attached income limits based on household size)
3. Be housed and responsible for paying the cost of their heating utilities
2006/2007 Heating Season Process
1. Ryan White Title II case managers should determine which of the clients they serve are on the OHOP wait list and potentially eligible for and in need of LIEAP assistance.
2. Ryan White Title II case managers should contact those potentially eligible clients to complete the attached LIEAP pre-screening form, collect a copy of income verification from the last month, and collect a copy of the client's current utility bill.
3. Ryan White Title II case managers should fax the completed LIEAP Pre-screening Form and accompanying documentation to their regional OHOP housing coordinator.
4. The OHOP housing coordinator will request additional information, if needed, via the Ryan White Title II case manager, and determine client eligibility.
5. If the client is eligible, the OHOP housing coordinator will make a LIEAP request and submit confirmation of the LIEAP request directly to the client (with a copy to the Ryan White Title II case manager).
Ryan White Title II case managers will take the lead role in identifying OHOP wait list clients in need of LIEAP assistance and in helping the client gather the information needed to process the request (via the attached Pre-screening Form). Ryan White Title II case managers should not refer clients to the OHOP program solely for LIEAP assistance.
