Patient Education Handouts

Health care providers can use these handouts to help explain specific topics to their patients and reinforce key messages of prevention and control of diabetes.   Some have places for the doctor or other health care professionals to write down information or instructions.

  • There is no charge for these materials, but a few restrictions apply.  What restrictions apply to using these materials?

     

  • Materials will be updated periodically. To help you check on whether you are using the most recent version, all materials include a date in the middle of the credit line at the bottom of the page.

     

  • New materials will be added from time to time. Check back often.

     

  • You can print in color or black and white. Each piece is provided in both color and b&w pdf versions.
