Provider Resources and Tools Workgroup (currently inactive)
To help improve care for patients with asthma, health care providers need tools and resources that will help them to diagnosis and better manage asthma. The goal of the Provider Resources and Tools Workgroup is to develop tools that can assist practitioners in delivering care that is in accordance with the NAEPP guidelines.

Oregon Asthma Resource Bank
Asthma is a complicated disease that requires constant self-management in between visits to the doctor. While there are many materials out there to support asthma self-management, they are of varying quality and accuracy. In response to the need for easily accessible, reliable materials, a collaboration of healthcare providers from throughout the state came together to create the web-based Oregon Asthma Resource Bank (OARB). On OARB you will find free, print-ready, clinically accurate, low-literacy, reliable asthma education handouts and provider tools based on the National Asthma Education and Prevention Program’s (NAEPP) Guidelines for the Diagnosis and Management of Asthma and the Guide to Improving Asthma Care in Oregon
Meeting minutes
The Physician Advisory Council on Asthma identified three tools to be researched and developed by this group in accordance with the NAEPP guidelines.
- A Self-Assessment form for patients to fill out while waiting for their clinic appointment. This tool can be both educational for the patient and help lead the provider through an appointment.
- An Asthma Pocket Guide for providers to carry in their clinic coats. This tool is a brief summary of the NAEPP guidelines so that providers who do not regularly see asthma patients are easily reminded of the diagnosis and management guidelines for asthma.
- A statewide Asthma Action Plan to be used universally between providers and schools. While many of these forms exist, they exist either for schools or providers. This workgroup will develop an Asthma Action Plan that will cover the needs of the provider, family and community partners, such as schools and day care centers.
