E-mail/File Network Access
 
To grant or change network (e-mail, file server, mainframe) access for new or current DAS employees please complete the user management form.

New User
 
Purpose: To grant PC or network (e-mail, file server, mainframe) access for new DAS employees (i.e. employee has just been hired.)

Tips: We recommend that you complete this form as soon as you know who the employee is so that we can get their access completed by the time they start working. Allow at least 72 hours notification.
  1. Have a copy of it to your workarea.
  2. Have the appropriate manager sign a printed copy and mail or fax it, or have the appropriate manager attach it to an e-mail to the TSC Help Desk, indicating their authorization for the change.

User Account Termination
 
Purpose: To revoke PC or network (e-mail, file server, mainframe) access for current DAS employees (i.e employment is being terminated; employee is leaving the agency; employee is on job rotation and does not require computer access. )

Tips: We recommend that you complete this form as soon as you know that an employee is going to be terminated or will be leaving. If an employee is being terminated, we recommend that you contact the TSC on the phone prior to sending the form. Allow at least 24 hours notification.
 
 

Division Transfer/Name Change
 
Purpose: To change PC or network (e-mail, file server, mainframe) access for new DAS employees (i.e. an employee is moving within the agency/division/section); an employee requires a name change)

Tips: Allow at least 72 hours notification. 
  1. Complete the User Management form (doc) and save a copy of it to your workarea.
  2. Have the appropriate manager sign a printed copy and mail or fax it, or have the appropriate manager attach it to an e-mail to the TSC Help Desk, indicating their authorization for the change.
