The Employee Information Packet is a summary of FMLA/OFLA policy and procedures and explains eligibility requirements.
Employees must generally give 30 calendar days notice for planned absences (paid or unpaid) related to FMLA and OFLA leave. If unable to request leave in advance due to an emergency or unforeseeable event, let DAS Employee Services know as soon as possible.
Employees must provide enough information so the agency can make a qualifying determination.

DAS Employee Services will require a completed Health Care Provider Certificate form for all FMLA/OFLA events except for that designated as Parental Leave. This form must be returned within 15 days from the date requested or your leave can be denied.
